CLINIPHARM SERVICES, under the direction of its principal, Jack H. Raber, Pharm.D. has consulted to pharmacies in the hospital, community, home care, and long-term care settings throughout the country since its inception in 1988.  From 1996 to 2002, Dr. Raber also served as a member of the Medical Advisory Panel of the Professional Liability Division of the Farmers Insurance Group, lending expertise in matters pertaining to pharmaceutical care, pharmacy practice, and pharmacy risk management.  He has spoken extensively on these topics to a variety of local and national audiences.

With over 30 years of experience at both the practice and administrative levels, Dr. Raber is available to provide assistance in the following areas:

* Clinipharm Services is also available to develop and administer customized medication error documentation and reduction programs for community, long-term-care, and homecare pharmacies that satisfy the legal requirements of the California Code of Regulations Division 17, Title 16, § 1711 ("Quality Assurance Programs").  These programs are available to non-California pharmacies if mandated by their respective state regulations.
